Qualifications

• Experience in case management, social services, or related work preferred.
• Strong documentation and organizational skills.
• Excellent communication and problem‑solving abilities.
• Ability to manage sensitive situations with professionalism and compassion.

Responsibilities

• Manage resident caseloads and coordinate individualized service plans.
• Monitor resident progress and evaluate case outcomes.
• Help residents work toward personal goals, wellness, and greater independence.
• Advocate for residents and assist with informed decision-making.
• Coordinate care and collaborate with outside agencies, service providers, and community resources.
• Maintain accurate case notes, incident reports, and resident documentation in Mission Tracker.
• Participate in behavioral support planning and disciplinary procedures when necessary.
• Respond appropriately to emergency situations and coordinate with law enforcement, fire, or mental health services when needed.
• Participate in team meetings and case review discussions.
• Assist with on-call coverage on a rotating monthly basis
