Job summary

Hope Valley Health & Wellness in Wasilla, AK seeks a dedicated Case Manager/Group Facilitator to support clients through treatment planning and care coordination, using evidence-based practices and CBT principles.

The role requires at least two years of experience with substance use disorders, CPR/First Aid, TB test or chest x-ray, strong organizational skills, and the ability to work under pressure in a team.

Qualifications

• At least two (2) years of experience working with individuals with substance use disorders preferred.
• Current CPR and First Aid certifications are required.
• Must have a current TB test or chest x-ray.
• Knowledge of the requirements for providing services needed to clients.
• Experience in electronic charting preferred.
• Experience with conducting client group sessions, and one on one sessions is required.
• Strong understanding of the principles and application of cognitive behavioral therapy (CBT).
• Knowledge and understanding of HIPAA and CFR-42 client confidentiality regulations.
• Excellent written and oral communication skills.
• Ability to work well and quickly under pressure both individually and as part of a team.
• Excellent project management skills to consistently meet deadlines.
• Strong organizational skills with superior attention to detail
• Excellent interpersonal skills

Responsibilities

• Participates in client treatment planning and continuum of care.
• Conducts group sessions as scheduled, and documents as required.
• Assists with modifying behavioral problems, as needed.
• Responsible for initial treatment assessments, as well as follow up assignments.
• Ensures all documentation regarding client care, treatment and incidents is completed promptly.
• Provides crisis intervention to clients, as needed.
• Maintain a positive therapeutic environment, providing individual and group counseling.
• Oversee and manage client incidents, ensuring they are addressed promptly and effectively, and ensure that incident reports are completed accurately and on time.
• Complete necessary community referrals and ensure continuity of care with other agencies as needed.
• Conduct assessments, screenings, treatment planning, and managing clients' progress.
• Other duties as assigned
