Job summary

Youth Health Associates is seeking a full-time Case Manager to support youth in a residential treatment program. You will coordinate intake, staffing, and discharge alongside clinicians, ensure timely medical, dental, and vision exams, and advocate for clients' well-being.

The role requires a Bachelor's in Social Work or related field, 1+ year in casework, and CPR/First Aid certification. Hybrid work model with competitive salary and benefits.

Qualifications

• CPR and First Aid certification or willingness to obtain.

Responsibilities

• Transport residents to appointments and maintain records.

Case Manager - Residential Youth Treatment CenterAre you passionate about supporting children and adolescents through meaningful casework and clinical coordination? YHA is seeking a full-time Case Manager to join our team and provide essential support to youth placed in our residential treatment program.In this role, you'll work directly under the supervision of the Program Director, ensuring high-quality care through coordinated services, accurate documentation, and advocacy for our clients' well-being.Key Responsibilities:Coordinate intake, staffing, and discharge processesMonitor and schedule required annual medical, dental, and vision examsCommunicate with medical designees, manage all appointment paperwork, and notify legal guardians or conservatorsTransport residents to necessary appointments and maintain accurate documentationSchedule psychological and psychiatric evaluationsFacilitate treatment team meetings and notify appropriate partiesCoordinate with caseworkers, probation officers, and schools regarding hearings, education, and care plansMaintain up-to-date and well-organized client records (intake forms, medical records, reports, incident documentation, etc.)Facilitate and oversee delivery of Transitional Living Services for those youth on caseload 14 years and older.Track medication and obtain psychotropic meds as directed by psychiatristsAct as liaison with Desoto ISD, enroll new students, attend ARD meetingsSupport administrative tasks and ensure general office organization and cleanlinessServe as the designated driver for GHA as neededWhat We’re Looking For:Bachelor's degree in Social Work (or related field)Minimum 1 year of experience in casework or youth services (2+ years preferred)Exceptional organizational and communication skillsProficiency in Microsoft Word, Excel, and general computer usageCPR and First Aid certified (or willing to become certified)Experience with trauma-informed care and working with emotionally disturbed youthWork Environment & Conditions:Mixed office and field work with flexible hours as neededMust be comfortable interacting with youth in a variety of settings, including during crisisPay:Full time (40 hours per week)Salaried position.Starting at 53K/year.Why Join YHA?Youth Health Associates is committed to supporting both staff and clients through evidence-based practices, trauma-informed care, and a collaborative team culture.
