Valley Hope of Atchison in Atchison, KS is seeking a Case Manager to join our dedicated team. You will coordinate continuing care plans and facilitate recovery-focused group sessions for patients.

The role requires healthcare experience and CPR certification; preferred candidates hold a bachelor’s or graduate degree in behavioral sciences, education, or social work and CM certification in the state of employment.

Qualifications

• One year or more experience in a similar position and/or healthcare environment.
• CPR certification is required.
• Bachelor’s or graduate degree in behavioral sciences, education, or social work is preferred.
• One year+ experience in substance abuse counseling industry is preferred.
• Certified as Case Manager in the state of employment is preferred.

Responsibilities

• Plans and assists with continuing care for assigned patients.
• Facilitates patient rehabilitation group sessions and educational workshops on recovery topics.
• Identifies needed recovery resources for housing, education, vocational development, mental health, community support, and medical care.
• Provides education regarding substance use disorders, mental illness and prevention to patients and families.
• Works with external parties including family, agencies, managed care organizations, and insurers.
• Records treatment activities and patient observations in records; completes paperwork timely.
• Completes all clinical documentation including discharge materials accurately and timely.

EDUCATION & EXPERIENCE:

Required:

• One (1) plus years of experience in a similar position and/or healthcare environment.
• Cardiopulmonary Resuscitation (CPR) certification.

Preferred:

• Bachelor’s or graduate degree in behavioral sciences, education, or social work.
• One (1) plus years of work experience in substance abuse counseling industry.
• Certified as Case Manager in the state of employment.

JOB SUMMARY:

• Works under the supervision and direction of the Program Director.
• Plans and assists with the implementation of continuing care for assigned patients.
• Facilitates patient rehabilitation group sessions and educational workshops on recovery topics.
• Identifies needed recovery resources for housing, education, vocational development, mental health, community support groups and medical care; provides assistance to patients in accessing those resources.
• Provides education regarding substance use disorders, mental illness and prevention to patients, family members, and significant others.
• Works with all relevant and interested external parties including family members, agencies, managed care organizations, and insurance companies.
• Records treatment activities and patient observations in patient records accurately and timely; completes paperwork requirements as needed in an accurate and timely manner.
• Completes all clinical documentation including discharge materials in an accurate and timely manner.
• Communicates with onsite supervisor about daily job duties and supervisory direction provided.
• Communicates with clinical supervisors, clinical staff, and medical staff regarding patients’ treatment and status.
• Logs all supervision as per state requirements.
• Maintains and demonstrates a current working knowledge of applicable counseling procedures and substance abuse treatment in accordance with company policy and State and Federal guidelines.
• Adheres to Valley Hope’s clinical philosophies, policies, and procedures as well as any other regulating bodies to ensure quality patient care.
• Participates in all assigned staff meetings, staff development, and training as required.
• Interacts and communicates with others (patients, co-workers, management, clients, and vendors) in a professional and tactful manner including treating them with respect and consideration regardless of their status or position.

WORK ENVIRONMENT:

• Residential treatment facility setting; requires walking throughout the facility.
