Greystone Foster Care is seeking a dedicated Case Manager to join their foster care program in a hybrid role blending remote work with in-office collaboration. The position supports at-risk youth, requires full-time dedication, and may include weekend and evening hours.

The successful candidate will hold a social work degree (Bachelor's required, Master's preferred) and possess a valid driver’s license. Prior experience and strong organizational skills are essential.

Qualifications

• Bachelor's degree in Social Work or related field; Master's degree preferred.
• One to three years' related experience and/or training, or equivalent.
• Valid state Driver's License and automobile insurance verification.

Responsibilities

• Complies with state regulatory requirements and Greystone Foster Care plan.
• Ensures placement documentation is completed within established timelines.
• Maintains regular file audits for each child and participates in audits.
• Assists with foster parent reassessments when necessary.
• Schedules quarterly child/family team meetings and documents accordingly.
• Visits assigned children per policy or as needed.
• Schedules EPSDT and dental appointments and communicates with DCS.
• Arranges outpatient mental health assessments for Level 2 youth and follows through.

Case Manager - Foster Care Program

The ideal candidate must reside in Tullahoma or Manchester, Coffee County, Tennessee.

Responsibilities

Universal Health Services, Inc. company, in partnership with Greystone Foster Care, has a long history in providing comprehensive behavioral health care services to at-risk youth struggling with extensive mental health issues. Greystone Foster Care recruits foster parents and offers the training to be equipped to provide a safe, stable and therapeutic environment for children and youth that have been removed from their homes. Our training prepares staff and foster parents to support children and youth as they begin the healing process. Our therapeutic foster parents link children and youth to hope, healing and trust.

Greystone Foster Care is seeking a dedicated, compassionate Case Manager to add to our growing team. This is a hybrid job blends remote work and in-office position with occasional requirements to report to the facility. This role requires full-time dedication and must be flexible to include weekends and evening work.

Job Duties/Responsibilities

• Complies with state regulatory requirements and Greystone Foster Care plan
• Ensures that all placement documentation is completed according to established timelines
• Completes regular file audits for each child and participates in regular peer file audits to ensure audit preparedness at all times
• Assists with completing foster parent reassessments when necessary
• Schedules child/family team meetings at least quarterly and corresponding documentation is completed/submitted
• Visit each assigned child per policy or as needed
• Ensures that annual EPSDT and biannual dental appointments are scheduled and information provided to DCS
• Ensures that outpatient mental health assessments are scheduled for all Level 2 youth and participation in recommended services occurs
• Obtains required consents and releases from appointed guardians for medical appointments and medications
