Marie Detty Youth and Family Services seeks a dedicated Case Manager to support shelter residents and ESG participants with case management, crisis intervention, and trauma-informed services.

You will document eligibility, maintain files, coordinate housing and employment referrals, deliver life skills training, and work with advocates and partners to connect clients to resources.

Qualifications

  • Bachelor's degree in behavioral health or related field is required.
  • At least 1 year of supervisory experience preferred.
  • Experience with domestic violence and sexual assault issues preferred.
  • Strong writing, verbal, and organizational skills; ability to manage a heavy caseload.
  • Ability to lift at least 25 lbs and maintain records confidentially.

Job description

ResponsibilitiesWill submit work orders for maintenance of shelter to maintain the buildings integrity and upkeep.Responsible for maintaining monthly schedule of events for residents.Must attempt contact with each resident within 5 business days of intake into shelter to complete individual service plan.Make referrals to community agencies and programs when applicable to include documentation of such services.Conduct initial evaluation to verify and document individual or family eligibility for ESG services.Collect and maintain all ESG paperwork in client files needed to meet HUD requirements.Conduct housing inspections for ESG participants.Provide at least once-a-month contact with each ESG program participant to monitor any barriers encountered by individual/family in obtaining or maintaining employment and/or housing.Ensure monthly reports are accurate and submitted in a timely manner for compliance with funders requirements.Input client data into Empower database or HMIS in a timely manner.Will provide crisis intervention over the phone and face to face and assist with crisis calls.Provide life skills training to clients in conjunction with other staff persons.Provide a forum where clients can air grievances or complaints without retaliation, for the purpose of conflict resolution and effective communication.Provide para-professional counseling when counselors are unavailable.Will assist client advocates in all their job responsibilities including sexual assault services and court advocacy, when necessary. This is to include being part of the Sexual Assault Response Team.Responsible for final review of client files and making sure information contained is complete, accurate and adheres to AG regulations.Responsible for attending all conferences, in-service training, and workshops to increase knowledge of domestic violence, stalking and sexual assault issues.Must adhere to all policy, procedure, and operations manuals.Maintain a clean, safe, and healthy environment.Assist with Client Advocate shift coverage when necessary.Provide educational workshops and presentations to the community about the agency and program services.Participate in events to educate community about DVSA and stalking.Responsible for completing the required minimum number of training hours per year (24 hours/full-time; 12 hours/part-time).Other duties as required by program needs.CompetenciesUnderstands and applies the agency policies and procedures (i.e. dress code and appearance, adherence to work schedules, completion of trainings, etc.).Punctually attends meetings and trainings. Completes assigned trainings in adequate time frames and obtains the minimum number of training hours required within the calendar year.Demonstrates dependability by consistently working scheduled shifts and following proper procedures when unable to work scheduled shifts.Displays cultural competency by valuing the fundamental rights, dignity, and self-worth of all people including respecting age, culture, disability, ethnicity, race, religion/spirituality, gender, sexual orientation, marital status, socioeconomic status, or any basis proscribed by law.Interacts with others in a professional manner and effectively navigates interpersonal differences. Maintains appropriate boundaries with supervisors, colleagues, and clients.Demonstrates the ability to consult or seek supervision regarding concerns or issues and communicates effectively with colleagues and clients.Displays professionalism and ability to work as part of a team.Identifies potential safety issues and takes measures to reduce risks and protect clients and staff.Understanding of trauma-informed services and sensitivity to the unique circumstances to survivors of domestic violence and sexual assault.Exhibits interaction with each new client for a minimum of five minutes to introduce yourself and the program to discuss services and case management needs.Shows ability to work collaboratively with residents and non-residents in identifying supports, challenges, and service needs to connect clients to community resources.Develops ongoing referral relationships with health, legal, housing, and other social service providers.Demonstrates ability to audit client records to ensure accurate billing and data entry in a timely manner.Exercises good judgement and handles sensitive information confidentially.The Case Manager will receive training and be evaluated based on each of the above competencies and responsibilities. Marie Detty Youth and Family Services is an at-will employer. Employment is by mutual consent of the company and employee and either may terminate the relationship at any time, with or without cause or advance notice. This job is contingent upon funding; however, availability of funding is not a guarantee of continued employment, nor does it imply any specific terms of employment.RequirementsMust be 21 years old.Bachelor’s degree in a behavioral health field. One year of supervisory experience preferred. Experience with domestic violence and sexual assault issues preferred.Demonstrate strong writing, verbal, and organizational skills.Must have strong assessment and interpersonal skills, flexible attitude, and able to meet the demands of a heavy caseload.Must be able to lift at least 25 lbs.Must have a valid driver’s license and telephone.Must pass FBI background investigation.Ability to communicate with and advocate on behalf of residents.Ability to maintain records and confidentiality.Ability to maintain a professional appearance and demeanor at all times.Ability to assess situations, identify potential safety issues, and independently initiate safe interventions.Must be able to attend mandatory trainings and monthly staff meetings as scheduled. The Case Manager is required to become certified (within 90 days) in CPR/First Aid.
