Job summary

Horizon Services, Inc. in San Leandro, CA is seeking a Case Manager to provide client-centered case management at the Detox Center.

You will coordinate medical, psychiatric, and continuing care appointments, document thoroughly, and collaborate with Alameda County behavioral health providers to support care and referrals. Candidates must hold California state certification or BBS registration, be comfortable with crisis intervention, and serve diverse populations.

Qualifications

  • Current registration or certification with CA Substance Use state certifying body (CAADE/CCAPP/CADTP), or registration with the California Board of Behavioral Sciences (BBS) as an AMFT or ASW.
  • Proficient in MS Word, Excel, PowerPoint and database systems.
  • Experience providing case management services in a mental health or substance use disorder program in the nonprofit sector.
  • Knowledge of demographic data collection.
  • Safeguard Protected Health Information (PHI).
  • Ability to manage time and prioritize.

Responsibilities

  • Provide case management services to clients at the Detox Center.
  • Schedule and arrange client medical, psychiatric, and or continuing care appointments.
  • Maintain appropriate notation and documentation in client file.
  • Maintain accurate documentation in accordance with case management billing practices.
  • Provide timely crisis intervention when needed.
  • Meet with clients to determine case management and referral needs.
  • Participate in all required consultation with Alameda County Behavioral health providers regarding client care and case management experience and opportunities.
  • Maintain positive working relationships with community providers and managed care organizations.
  • Assist with case management training at staff meetings.
  • Provide clients with educational materials and orientation of continuing care options, including withdrawal management, mental health services, residential treatment, and recovery residences.
  • Demonstrate good decision making on a daily basis.
  • Utilize email and other forms of technology to communicate with peers, supervisors, and community members.
  • Attend all scheduled team meetings and trainings.
  • Assist with data entry in to EHR system as needed.
  • Drive program vehicle when necessary.
  • Perform any other duties as assigned by Program Manager.

Job description

Bonus for Bilingual Applicants: $2,000 (Distributed in 4 parts: $500 after 30 days, $500 after 90 days, $500 after 150 days, and $500 after 210 days of employment). The bilingual bonus applies to client-facing positions and will be awarded upon successful completion of the language test.

What will make you successful in this role?

  • Able to exercise good judgment and maintain healthy boundaries with participants.
What else is required?
  • Valid California driver’s license with a good driving record and car insurance.
  • Current CPR/First Aid certificate desirable.
  • Background Check/drug testing required at time of hire
  • Current flu vaccine/flu shot
  • TB Test

Salary:
