Lambs Farm is a lakeside community in Libertyville, IL, dedicated to empowering adults with developmental disabilities through meaningful work and supportive services. We seek a compassionate Case Manager to oversee a caseload of 12, coordinate across vocational, recreational, health services, and engage families and guardians in person-centered care.

Certification requirements include QIDP eligibility and CPI/NCI training with Lambs Farm.

Formación

• Bachelor’s degree in social services, healthcare, psychology, or a related field.
• Minimum 1 year of experience working with individuals with developmental disabilities.
• QIDP certification or the ability to complete within 6 months of hire.
• Knowledge of DHS, IDPH, HIPAA, Medicaid and related guidelines.
• Valid driver's license with a clean driving record.
• CPR, First Aid, CPI or NCI certifications through Lambs Farm program.
• Complete Lambs Farm DSP program within 120 days of hire.

Responsabilidades

• Conduct assessments to tailor support plans to participant goals.
• Develop and monitor outcome-based services ensuring compliance with state/federal guidelines.
• Facilitate annual planning and interim check-ins; summarize monthly progress.
• Provide crisis intervention in collaboration with Behavioral Analysts.
• Serve as liaison for families, guardians, ISC, and external stakeholders.
• Train Direct Support Persons in service plan implementation and documentation.
• Assist with financial and insurance concerns as needed.
• Support community engagement on- and off-campus.
• Attend and present cases to Behavioral Support and Human Rights committees.
• Follow up on behavioral incidents per DHS and IDPH requirements.
