Job summary

Pilsen Wellness Center is seeking a dedicated Case Manager to participate in implementing the individual’s service plan to boost independence in daily living, economic self-sufficiency, and community integration.

The role is based in Chicago, IL, full-time, with a salary of $45,000 per year. English/Spanish bilingual abilities and strong documentation skills are preferred.

Qualifications

• Minimum high school diploma or GED required.
• Active driver's license is required.
• Bilingual English/Spanish preferred.
• 3–5 years in administrative/clerical support.
• Cultural humility and ability to build trust with diverse populations.
• Strong documentation and collaboration skills.

Responsibilities

• Assist clients in achieving independence in housing and finances.
• Support with employment/education as needed.
• Maintain timely and accurate clinical documentation.
• Help participants access services in their service plans.
• Adhere to ethical standards and confidentiality.

Monday - Friday, 12:00PM - 8:00PM OR Tuesday - Friday 12:00PM - 8:00PM and Saturday 9:00AM - 5:00PM

Job Summary

Pilsen Wellness Center is seeking a dedicated and compassionate Case Manager to participate in the implementation of the individual’s service plan, specifically designed to increase an individual’s skills in independence in daily living, economic self-sufficiency and community integration.

Responsibilities

• Assists the client in achieving independence (e.g., housing, finances)
• Assists with supported employment/education as needed
• Maintain timely and accurate clinical documentation
• Helps participants to access the services specified in their service plans
• Adhere to ethical and confidentiality standards

Qualifications

• High School Diploma or GED
• Active Drivers License
• Bilingual (English/Spanish) preferred
• 3-5 years of administrative and clerical assistance experience
• Cultural humility and the ability to build trust with diverse populations
