Job summary

Henry Street Settlement is hiring a Case Manager to deliver participant-centered support aimed at housing stability, employment, and long-term self-sufficiency. You will assess needs, develop action plans, coordinate services, and help participants access financial and community resources.

The role requires two years of case management experience, a Bachelor’s degree in social work or related field, and strong collaboration with a multidisciplinary team in a dynamic urban setting.

Qualifications

• Bachelor’s degree in social work or related health/social services discipline.
• Minimum of two years of case management experience serving individuals or families facing significant barriers.
• Experience with NYC housing navigation, supportive housing, homelessness prevention, or services for special-needs populations.
• Experience working with young adults and/or families experiencing housing instability is strongly preferred.
• Ability to assess needs, develop action plans, coordinate services, and support participants through crisis and stabilization.

Responsibilities

• Case Management & Individualized Planning: Manage caseload, conduct intake, screening, assessments, and develop self-sufficiency plans; maintain regular contact.
• Housing Stabilization & Crisis Prevention: Identify housing needs and coordinate interventions to prevent or resolve crises.
• Financial & Supportive Services: Assess eligibility for financial assistance and connect to benefits, childcare, food, healthcare, and counseling.
• Employment & Economic Mobility: Provide job-search assistance, training referrals, and connect to opportunities.
• Service Coordination, Documentation & Follow-Up: Maintain case notes, referrals, and collaborate with internal/external partners.
