Job summary

Oaklawn Psychiatric Center is hiring Case Managers in Elkhart or Goshen to support youth, adults, and families in achieving goals and improving wellbeing. You will evaluate needs, develop treatment plans, and connect clients with housing, food, medical care, and transportation.

Roles involve collaboration with DCS, probation, schools, therapists, and community agencies, with casework documentation and occasional evening/weekend hours.

Qualifications

• BA/BS in social service or human services-related field required.
• Two years’ experience in a social service setting preferred; alternative is an associate degree with two years of direct casework experience.

Responsibilities

• Evaluate consumer needs and develop/monitor treatment plans.
• Help consumers obtain services such as housing, food, medical care, and transportation.
• Advocate for families and coordinate with DCS, courts, schools, and partners.
• Document interactions and maintain case notes in electronic systems.
• Maintain caseloads with consideration for training, complexity, and availability, including evenings and weekends.
• Work directly with probation and Department of Child Services, crisis services, and Wraparound programs.
