Job summary

Oaklawn Psychiatric Center in Northern Indiana invites you to join our Case Manager team serving South Bend and Mishawaka. You’ll help people with mental health and substance use challenges set goals, access services, and build healthier, more independent lives.

Qualifications

• Associate degree and two years of experience in a similar human services job OR
• BA/BS in social service or a human services-related field preferred
• Two years’ experience in a social service setting preferred
• Must have a valid driver’s license and proof of insurance

Responsibilities

• Case management for youth, adults, and families to support needs and goals.
• Coordinate services across Child & Adolescent programs, Addiction Services, School Liaison, and Intensive Services.
• Provide case management and skills training; some evening hours may be required.
