Job summary

Damar, located in Indianapolis, IN, is seeking a Case Manager to lead, direct and monitor treatment services for individuals with developmental disabilities or autism, while promoting the agency philosophy in service delivery.

Responsibilities include developing IPPs within 7 days of admission, coordinating with Residential Managers, maintaining notes and data, and leading program meetings to ensure high-quality care for clients. Competitive starting salary and strong benefits are offered.

Qualifications

Must have at least one year of experience treating DD/ID population or specialized training.
Minimum age of 21 per licensure requirements and valid Indiana driver’s license.
Be clear of felony convictions and successfully complete pre-employment physical and background screening.

Responsibilities

Represent clients at doctor’s appointments and exchange information accurately.
Develop IPPs for each client within 7 days of admission and update as needed.
Plan and lead program meetings and collaborate with Residential Managers.
Write and update monthly IPPs to be ready by the 6th of each month.
Monitor IL goals and complete IL monthly reports.
Ensure family and referral source communication weekly.
Attend quarterly CFTM or as needed and participate in court reviews/IEP conferences.
