Job summary

Qualifications

• Experience working with DD/ID populations or specialized training.
• Must have valid Indiana driver’s license and acceptable driving record.
• Successful completion of a pre-employment physical and background screening.

Responsibilities

• Lead, direct and monitor treatment services for assigned clients.
• Develop and update IPPs within 7 days of admission and monthly thereafter.
• Represent clients at appointments, Reviews and conferences with accurate information.
