Job summary

Telecare Corporation seeks a compassionate case manager to support individuals with mental illness and co‑occurring disorders in living independently in their communities. Under supervision, you will provide direct services and help implement behavioral plans, coordinating with a multidisciplinary team.

You must have 1+ year of direct service experience with dual diagnosis, and be CPR/CPI/First Aid certified or willing to obtain within 60 days.

Qualifications

• High School graduation or GED with 3 years direct services in mental health or community services, or an associate’s degree with 2 years, or a bachelor’s degree in Social Services.
• At least 1 year of direct service experience with dual diagnosis communities, creating and executing behavioral plans, and working with individuals with impaired or limited cognitive functioning.
• Sensitivity to multicultural populations and issues.
• Minimum age 18.
• CPR, Crisis Prevention Institute (CPI), and First Aid certification by employment date or within 60 days, maintained throughout employment.
• Valid, current driver’s license and personal vehicle insurance with your name listed as a driver.
• Willingness to use personal vehicle to drive clients to appointments and groups, with weekly mileage reimbursement at the IRS rate.
• Willingness to use company vehicle for client transport.
• Willingness to use personal vehicle to attend meetings, with weekly mileage reimbursement at the IRS rate.
• Successful completion of post‑offer physical examination, 2‑step PPD test for tuberculosis, background checks, excluded party sanctions, and license verification; and for driving positions, a motor vehicle clearance and proof of auto insurance.

What You Will Do to Change Lives

Full‑Time | AM | 8:00 AM – 5:30 PM | Monday – Friday

Starting wage range is $28.27 – $34.54. The pay range is based on location and is commensurate with experience beyond the minimum requirements.
