I. Major Responsibilities:

1. Performs comprehensive case management intake assessment and develops plans with clients for obtaining services needed.
2. Completes initial behavioral health intake documentation as required by the program, payor, and regulatory bodies.
3. Works collaboratively with the integrated team to coordinate the delivery of appropriate behavioral health and other support services.
4. Provides ongoing case management services to assist clients with obtaining various entitlements and benefits.
5. Provides transportation assistance to individuals as needed.

II. Position Qualifications:

License/Certification/Education:

1. Required: High school diploma/GED.
2. Required: Satisfactory CORI background check.
3. Preferred: Associate’s degree.

Experience/Skills:

1. Required: Ability to develop a working knowledge of CHL’s service lines.
2. Required: Excellent oral communication and computer skills.
3. Required: Ability to multitask and manage time effectively.
4. Required: Professional integrity and cultural competency.
5. Required: Ability to use office equipment.
6. Required: Ability to organize complex tasks and meet deadlines.
7. Preferred: Prior experience in a behavioral health or social services setting.
8. Preferred: Basic knowledge of CHL services and insurance requirements.
9. Preferred: Working knowledge of Electronic Health Records (EHR).

Unless certification, licensure, or registration is required, an equivalent combination of education and experience may be substituted for the above requirements.
