Exemption Status: Non-Exempt

Schedule Details: Friday, Saturday, Thursday, Tuesday, Wednesday

Scheduled Hours: 8:30am-5pm, with 1 day per week 12pm-8:30pm

Shift: 4 - Mixed Shift, 8 Hours (United States of America)

Hours: 40

Cost Center: 71000 - 0125 Detoxification

I. Major Responsibilities:
  1. Performs comprehensive case management intake assessment and develops plans with clients for obtaining services needed.
  2. Completes initial behavioral health intake documentation as required by the program, payor, and regulatory bodies.
  3. Works collaboratively with the integrated team to coordinate the delivery of appropriate behavioral health and other support services, including coordinating supportive transitions in care providers.
  4. Provides ongoing case management services to assist clients with obtaining various entitlements and benefits (e.g., SSI, EAEDC, MassHealth, addiction services, and other community resources).
  5. Provides transportation assistance to individuals as needed and assists clients in accessing transportation through existing or new resources.
  6. Ensures that after-care needs of clients are met through appropriate referrals for community-based services, including substance use disorder, mental health, medical, legal, and social services, and coordinates ongoing care plans.
  7. Provides outreach to community-based residents as designated.
  8. Dispatches into the community with a clinician to support AMCI and YMCI clinicians as needed.
  9. Supports both Adult Mobile Crisis Intervention (AMCI) and YMCI Youth Mobile Crisis Intervention programs as needed.
Standard Staffing Level Responsibilities:
  1. Follows departmental policies, procedures, and objectives.
  2. Attends meetings, conferences, seminars as required.
  3. Uses Quality Improvement in daily operations.
  4. Complies with health and safety regulations.
  5. Respects diverse views, demonstrates professionalism, tolerance, civility, and acceptance.
  6. Maintains reliable and predictable attendance.
  7. Performs other duties as required.
II. Position Qualifications:

License/Certification/Education:

Required:

  • High school diploma/GED
  • Satisfactory CORI background check

Preferred:

  • Associate’s degree
Experience/Skills:

Required:

  • Ability to develop a working knowledge of CHL’s service lines for appropriate triage.
  • Excellent oral communication and computer skills.
  • Ability to multitask, prioritize, and follow through.
  • Personal and professional accountability, self-management, integrity, and cultural competency.
  • Ability to operate office equipment.
  • Organize complex tasks and meet deadlines.

Preferred:

  • Experience in behavioral health or social services preferred.
  • Knowledge of CHL services and insurance reimbursement.
  • Working knowledge of Electronic Health Records (EHR).

Equivalent education and experience may substitute for requirements where applicable.

III. Physical Demands and Environment:

Requires travel to facilities and residences; primary work indoors in various environments.
