SSM Health Day Institute in Olivette, MO is seeking a Case Manager to work in our outpatient rehabilitation program with the ability to float to other Day Institute facilities across the St. Louis region.

Under general direction, you will monitor and assess patient plans from pre-admission to post-discharge, assist families in goal setting to maximize independence, and liaise with payers to obtain necessary authorizations for initiating and continuing care throughout admission to the program.

Qualificações

• One year medical rehab case management experience (preferred).
• Professional Healthcare Degree in either Nursing or Social Work.
• Licensed PT, OT, SLP preferred.

*Full-time (M-F 8am-4:30pm)*

Experience in physical medicine and rehabilitation strongly preferred.

SSM Health Day Institute is seeking a Case Manager to work in our specialized outpatient rehabilitation program and have the ability to float within the region to assist other Day Institute facilities. We have ten centers across the St. Louis Metro area serving individuals recovering from a traumatic injury or illness who no longer require 24-hour nursing or acute rehabilitative care. Responsibilities

Qualifications

Professional Healthcare Degree in either
