Comprehensive Healthcare seeks a Case Manager to support outpatient clients across crisis and community settings. The role involves home visits, scheduling and coordinating services, and partnering with treatment teams to develop individualized plans.
Travel throughout the local area is expected. Qualifications include a Bachelor's degree in social work or related field, eligibility for Agency Affiliated Counselor registration, and a valid driver's license with insurance.

Case Managers work within various teams at Comprehensive to provide support and coordination of care to our outpatient clients. Recognizing the complex needs beyond mental health and substance use treatment, Case Managers work with clients to build skills necessary for independent living, self-advocacy, treatment engagement, and social participation. Case Managers support clients and ensure they are connected with the necessary community resources that will enable them to fully engage with and be successful in treatment. To be successful in this role, Case Managers will need excellent communication and relationship-building skills, knowledge of community resources, and comfort with making home visits and traveling locally throughout the workday. This position is part of our crisis team and may work with individuals of any age in crisis. This position provides support and case management services to those seen in crisis placed at the crisis triage center. In addition, this person may provide complete other tasks such as working with walk-in crisis clients, completing crisis follow ups, and going out on crisis outreaches to the community with other crisis staff. Frequent travel throughout the local community is required as part of this position.
