Housing Works in New York, NY is seeking a Case Manager for the DYCD-backed CRED program. You will conduct screenings, intake assessments, co-develop service plans, and connect clients to wraparound health services.
The role emphasizes documentation, benefits enrollment, and retention support within an interdisciplinary team; travel within boroughs is required and bilingual Spanish is preferred.
In partnership with the Department of Youth & Community Development (DYCD), Housing Works launched the CRED Program, a new specialized 24-week paid training program for justice-involved and justice-impacted individuals ages 25 - 40. The program provides case management, connection to wraparound health services, industry recognized credential trainings, paid internships and employment placement and retention support.
Reporting to the Program Manager, the Case Manager will be responsible for, but not limited to, conducting screenings and intake assessments, co-developing service plans, connecting individuals to wraparound healthcare services, assisting with benefits and entitlements enrollment, providing program retention support and utilizing the agency and funder’s electronic records systems to document client engagement and program outcomes.
