Family & Children's Center in La Crosse, WI is seeking a Case Manager for our Respite Program to help connect children, caregivers and respite providers with needed services. You will assess, plan, coordinate resources, and advocate for families while building strong, collaborative relationships.

Responsibilities include developing service plans, coordinating community resources, maintaining records, and ensuring program compliance.

Qualifications

  • Associate's degree required; Bachelor's degree in Social Work, Human Services, Psychology, Sociology or related field preferred.

  • Strong communication and problem-solving abilities are essential.

What You'll Do:

As a Case Manager for our Respite Program, you'll help connect children, caregivers, and respite providers with the services and support they need to thrive. This is more than a case management job - it's an opportunity to reduce caregiver stress, strengthen families, and create meaningful moments of relieve and connection. Respite services provide temporary relief for youth and primary caregivers of youth with disabilities, chronic illnesses, or other special needs.

In this role, you'll serve as a trusted partner for individuals and families by:

  • Conducting assessments and determining eligibility for respite services
  • Developing and monitoring individualized service plans
  • Coordinating services and connecting families with community resources
  • Supporting caregivers by identifying needs and advocating for appropriate services
  • Building supportive relationships with caregivers, children and respite providers
  • Maintaining accurate documentation and case records
  • Monitoring program compliance and service quality
  • Collaborating with community partners, providers and interdisciplinary teams
  • Providing ongoing support to help families achieve their goals and improve quality of life

QualificationsWhat We're Looking For
  • Associate's degree required; Bachelor's degree in Social Work, Human Services, Psychology, Sociology or a related field preferred.
  • Experience working with children, families or individuals with support needs is highly valued
  • Strong communication, relationship‑building, organization and problem‑solving skills
  • Ability to manage multiple priorities with empathy and professionalism
  • Compassionate and committed to serving others
  • Comfortable working independently while collaborating with a team
  • Knowledgeable about community resources and person‑centered services
  • Valid driver's license and ability to travel locally as needed
  • Experience in case management, human services, healthcare, disability services or a related setting preferred
