Job summary

Family & Children's Center in Wisconsin is seeking a Case Manager for the Respite Program to connect children, caregivers, and respite providers with services and support. This role helps reduce caregiver stress and strengthen families by coordinating resources and advocating for appropriate services.

The position emphasizes compassionate, person-centered care, collaboration with teams, and maintaining accurate records to ensure high-quality service delivery and outcomes for families we serve.

Qualifications

  • Associate's degree required; Bachelor’s degree in Social Work, Human Services, Psychology, Sociology or related field preferred.
  • Experience working with children, families or individuals with support needs is highly valued.
  • Strong communication, relationship-building, organization and problem-solving skills.
  • Ability to manage multiple priorities with empathy and professionalism.
  • Compassionate and committed to serving others.
  • Comfortable working independently while collaborating with a team.

Responsibilities

  • Conducting assessments and determining eligibility for respite services.
  • Developing and monitoring individualized service plans.
  • Coordinating services and connecting families with community resources.
  • Supporting caregivers by identifying needs and advocating for appropriate services.

  • Monitoring program compliance and service quality.

What You\'ll Do:

As a Case Manager for our Respite Program, you\'ll help connect children, caregivers, and respite providers with the services and support they need to thrive. This is more than a case management job - it's an opportunity to reduce caregiver stress, strengthen families, and create meaningful moments of relieve and connection.Respite services provide temporary relief for youth and primary caregivers of youth with disabilities, chronic illnesses, or other special needs. In this role, you\'ll serve as a trusted partner for individuals and families by:

  • Conducting assessments and determining eligibility for respite services
  • Developing and monitoring individualized service plans
  • Coordinating services and connecting families with community resources
  • Supporting caregivers by identifying needs and advocating for appropriate services

What We\'re Looking For

  • Valid driver\'s license and ability to travel locally as needed
  • Experience in case management, human services, healthcare, disability services or a related setting preferred
