Qualifications

  • Experience working with individuals with mental illness or related field.
  • Bachelor's degree preferred or 2 years of relevant experience.
  • Current Minnesota Driver’s License and automobile insurance required.

Base pay range

$50,000.00/yr - $50,000.00/yr

The Role: Community Health Case Manager Reports to: Manager of Vail Connect and A.O.S FLSA Status: Exempt

Salary Range: $50,000+ based on experience

What will you do?

Benefits

The Navigator supports individuals with serious mental illness - often accompanied by substance use disorders, chronic medical conditions, and frequent hospitalizations - by connecting them to essential community resources that promote safety, stability, and recovery. These include housing, mental health and substance use treatment, medical care, insurance, waiver benefits, and support for education and employment. The Navigator ensures continuity of care through referrals, service coordination, discharge planning, and individualized goal setting. Some but not all the duties include:

  • Provide intensive, assertive outreach to locate and connect with referred individuals.
  • Complete required paperwork and provide referrals and case coordination for individuals receiving services, including mental health, medical, financial, social, vocational, housing, and legal services.
  • Complete required documentation, including assessments, crisis plans, releases, and Individual Goal Plans on schedule, per performance standards.
  • Provide crisis assistance and intervention, including use of community crisis services.
  • See all individuals assigned based on their current needs; prioritize the needs of all individuals receiving services and increase or decrease level of support when needed.
  • Co-facilitate housing support groups as scheduled.
  • Assist in developing and implement a plan to achieve the desired outcomes, such as housing supports, housing search, and/or specialized assistance programs.
  • Reduce the risk of losing housing by providing assistance to access emergency assistance, housing funds, and ongoing support services.
  • Assist applicants through application processes for long term support.
  • Apply knowledge of tenant and landlord rights and responsibilities and educate and advocate for individuals as needed.

Candidates must have either two years of experience working with individuals with mental illness; or, have earned a bachelor’s degree in human services or related field along with at least one year of professional or internship experience in the mental health field. Individuals with a Social Work degree must hold valid and current Social Work licensure. Community Health Workers are encouraged to apply. A valid Minnesota Driver’s License, current automobile insurance, and reliable access to a vehicle are required for attending meetings in the office and community; mileage reimbursement is provided.
