ESSENTIAL DUTIES & RESPONSIBILITIESHelp patients navigate the health care system through coaching, education and supportCoordinate the behavioral and primary care needs and develop strategies to remove barriers to wellnessRefers, advocates, and links client and family for additional services as identified.Use behavioral management techniques, crisis intervention, active listening and motivational interviewing to help patients develop better life management skillsProvides supportive and educational case management services to develop daily living and coping skills.Responsible for developing, implementing, evaluating, and updating treatment plans with input from the client and family.Enters written documentation into Electronic Medical Record in a timely manner for all services rendered.Connect with clients face to face in the office and community, telehealth, and by phone to address needsDemonstrate knowledge and understanding of ethical standards by evidence of practice with patients and community partners.Attend weekly staffing and team meetings and completes assessmentsAssist clients with developing natural support systems.Completes all required agency trainings to maintain 100% compliance.Maintains departmental productivity expectations.Attend and participate in clinical supervision.KNOWLEDGE, SKILLS & ABILITIESQUALIFICATIONSEducation LevelBachelor’s Degree in psychology, social work or related field required.Years of ExperienceExperience with Electronic Health Record preferred.Certification/LicensureValid driver’s license in good standing required.PHYSICAL REQUIREMENTSStanding – 10%Sitting – 90%Squatting – OccasionalKneeling – OccasionalBending – OccasionalDriving – OccasionalLifting – OccasionalDISCLAIMERThe duties and responsibilities described in this document are not a comprehensive list and additional tasks may be assigned to the employee from time to time, or the scope of the job may change as necessitated by business demands.Time Type:Full timePay Range:$20.05-$28.10Delivering care that changes people's lives starts with our employees.
