Job summary

WestCare Pacific Islands Inc. in Guam seeks a Case Manager (Temp) to coordinate services and develop individualized plans for clients using a person-centered approach. Qualifications

  • Graduation from an accredited four-year college with a related degree.
  • Two years of case management experience preferred; intake/assessment experience preferred.
  • Strong communication and ethical conduct; ability to work in a team.

Responsibilities

  • Case management of a large caseload (up to 30 participants) with ISP planning.
  • Link clients to community resources and provide warm handoffs.
  • Heavy documentation and maintenance of client files.
  • Attend Guam Homeless Coalition Continuum of Care meetings and trainings.
  • Advocate for youth and families at court, school and Wrap team meetings.
