Job summary

Gulf Bend Center in Victoria, TX seeks a Child and Adolescent Services Case Manager to provide case management, skills training, and medication training in school, community, or clinic settings.

The role requires a BA/BS in a social, behavioral, or human services field (or RN license) with preferred experience in mental health services for children and adolescents; travel and driving are required. Monday–Friday, 8am–5pm on-site.

  • Submit all documentation in the EHR within two business days.
  • Meet productivity standards and use curricula with fidelity.
  • Track caseload to avoid lapses in consent and forms.
  • Participate in team meetings and supervision for professional growth.

JOB TITLE:

Child and Adolescent Services Case Manager

WORKSITE LOCATION:

Gulf Bend Regional Plaza, 6502 Nursery Drive, Suite 100, Victoria TX 77904

STARTING PAY:

$18.47/HR EEOC CODE: Professional FLSA STATUS: Non-Exempt

SHIFT:

Fulltime; Monday through Friday, 8:00am to 5:00pm

JOB PURPOSE:

Provides case management, skills training, and medication training to children/adolescents with serious emotional disturbance and services to the primary caregiver or LAR, in the school, community and/or clinic.

EDUCATION AND EXPERIENCE:

Required: A bachelor's degree from an accredited college or university with a major in social, behavioral or human services or is a registered nurse. (Social, behavioral, and human services are psychology; social work; medicine; nursing; rehabilitation; counseling, sociology; human development; physician's assistant; gerontology; education psychology; special education; early childhood intervention; or early childhood education.) Preferred: At least six months' experience in providing mental health services to adults, children and adolescents.

DEGREE AND/OR CERTIFICATION REQUIRED:

BA/BS in field specified above or licensed as a Registered Nurse in the State of Texas.

ESSENTIAL FUNCTIONS and DUTIES:
  • Completes TRR assessment in accordance with Center policy and HHSC. Initial TRR assessments are completed within 7 days of assignment and must be updated at least two weeks prior to expiration for continuity of care.
  • Formulates a Recovery Plan to identify the appropriate skill areas, frequency and duration of services to be provided in conjunction with the consumer/families and based upon the TRR assessment according to TAC code, HHSC Contract, and Center policy.
  • Provides authorized Case Management services to consumers and their support according to the Recovery Plan developed using independent clinical judgment in accordance with Medicaid Rule, HHSC Contract, and UM Guidelines.
  • Provides ninety-five percent (95%) of services in the consumers' natural environment (including at home, school, and community).
  • Submits treatment recommendations through Utilization Review based on clinical necessity per supervisor review.
  • Submits all documentation in electronic healthcare record (EHR) including forms, services, disclosures, telephone contacts etc. within two (2) business days.
  • Meets productivity standard.
  • Uses assigned curriculums effectively and according to fidelity when providing skills training.
  • Tracks and manages caseload to avoid expiration of Consumer's required documents and forms to avoid lapses in services.
  • Actively participates in all team meetings and supervisions as a means of enhancing professional growth, reviewing and processing the provision of case management services, and appropriately addressing concerns and other requests as needed.

  • Program Specific: Knowledge of child development and mental health. Ability to engage children and families in treatment. Use sound judgment in the release of information as evidenced by lack of complaints or consumer rights findings.

  • Fleet: Completes fleet checklist as required. Report all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance.
OTHER REQUIREMENTS OR CONDITIONS:
  • Current State of Texas Driver License or if you live in another state, must be currently licensed in that state. Liability insurance required.
  • Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Safe driving record as defined by Gulf Bend Center.
  • Must have computer experience to include Microsoft Word, Excel (beginner to intermediate level), Outlook, Teams, and Zoom. Ability to type 40 wpm. Ability to operate office equipment (copy machine, fax). Ability to type 40 wpm.
  • Must complete all training requirements prior to the due date. Attend required training meetings. Relias training and any other training required for the role is 100% compliant.
  • Must maintain regular and reliable attendance, including compliance with the telework agreement. This is a full-time position. Days and hours of work are Monday through Friday, 8:00am to 5:00pm or agreed upon hours 100% of the time, to ensure consistency and completeness of the program's processes.
