Job summary

Gulf Bend Center is seeking an Outreach Nurse (LVN) to join our team in Victoria, TX. The role emphasizes person-centered, family-centered, and trauma-informed care while collaborating with the interdisciplinary treatment team to deliver holistic services.

Requirements include graduation from an accredited nursing program, a Texas LVN license, and at least three years of nursing experience; hospital or home health background is preferred.

Responsibilities

  • Provide quality services to customers in a community setting with minimal supervision.
  • Conduct a comprehensive assessment of the customers' environment, physical and mental wellbeing.
  • Administer psychotropic medications as prescribed, monitor for adverse reactions.
  • Gain and maintain knowledge of AIMS assessment technique with assessments every 90 days for applicable customers.
  • Gain and maintain knowledge of venipuncture performance; monitor lab orders and complete as needed.
  • Provide pill packing, education and supervision to customers as required.
  • Meet productivity of 60 hours of face-to-face contact each month.
  • Collaborate with all members of the treatment team to ensure best possible customer outcomes.
  • May perform other duties as assigned by the Director of Nursing Services.

JOB PURPOSE:

The Outreach Nurse is a Licensed Vocational Nurse (LVN) capable of practicing in a person-centered, family-centered, and trauma-informed manner. The Outreach Nurse will work closely with the other members of the interdisciplinary treatment team to achieve a wholistic approach to care.

DEGREE AND/OR CERTIFICATION:

Required: Graduation from an accredited school of nursing; a current, uninfringed license to practice vocational nursing (LVN) in the State of Texas; at least three years' experience in nursing.

Preferred: Hospital or home health experience.

  • 2.Providequality services to customers in a community setting with minimal supervision.
  • 3.Conducta comprehensive assessment of the customers' environment, physical and mentalwellbeing.

  • 10. Fleet: Completes fleet checklist as required. Reports all fleet problems directly to the Program Director within 24 hours of identifying problem. Uses GeoTab key fob in fleet vehicles and maintains road safety compliance. Must be insurable by Center's liability carrier if employee operates a Center vehicle or drives personal car on Center business. Safe driving record as defined by Gulf Bend Center.
