Job summary

Auberle’s Employment Institute is seeking an experienced Case Manager to support young adults ages 18-24 across Central Pennsylvania. You will screen and assess participants’ skills, help develop work ethic and address barriers, and coordinate with employers to place participants in career-track employment with benefits.

You will travel roughly 75% of the time to meet participants, conduct home visits, and monitor progress while maintaining detailed records in CWDS in compliance with policies

Qualifications

• Bachelor’s degree preferred, but related experience working with young adults would be accepted.
• ACT 33/34 and FBI Clearances
• Valid PA driver's license, vehicle, and proof of insurance
• Reliable transportation for travel across multiple counties (approximately 75% of the time).

Responsibilities

• Manage a caseload of program participants across two counties, conducting eligibility assessments, collecting paperwork and vital documents, making home visits, and delivering one-on-one case management services to identify skills, interests, and barriers to employment.
• Travel 75% of the time to meet participants in person, support their career readiness, and ensure engagement in the program.
• Assist participants in developing strong work ethic, soft skills, and other necessary competencies that will increase their employability and career advancement potential.
• Help participants identify and overcome personal or professional barriers that prevent them from achieving their employment goals.
• Develop, implement, and monitor individualized case plans and track progress toward goals and provide ongoing guidance.
• Serve as a dynamic role model and mentor, demonstrating leadership and support throughout the participants’ journeys.
• Collaborate with Auberle staff, employer partners, and community organizations to find innovative solutions to help participants secure high-quality jobs with benefits.
• Engage with local businesses to foster partnerships and facilitate job placements for participants.
• Maintain detailed and accurate records of participant progress, documentation of services, and case management activities in compliance with Auberle’s policies and the Commonwealth Workforce Development System (CWDS).
• Attend team meetings and engage with employer partners to share best practices and strategies for successful program outcomes.

Position Title and Overview:

Case Manager: Employment Institute

The Employment Institute, Auberle’s nationally recognized workforce development program, places unemployed and underemployed individuals in career track employment with benefits, impacting future generations. We are embarking on an exciting expansion in the Central Pennsylvania Region. The Employment Institute Case Manager is a mission driven person excited to support the goals and benchmarks of our young people (ages 18-24) as they embark on career journeys in the healthcare field. Case managers assist in the screening and assessment of a program participants’ skills and areas of interest for careers. The Case Managers will also work closely with the participants in developing work ethic, soft skills and addressing any other barriers that exist that prevent the participants from reaching their goals. Case Managers must demonstrate strong mentoring skills for the individuals that they will be working with. In addition to serving as a dynamic role model, this position will also form good working relationships with community partners and employers to help participants maintain their employment.
