Qualifications

• Bachelor or higher in a human services field.
• Eligibility for Michigan Social Worker Licensure or QMRP/QIDP.
• Valid driver’s license required.

Responsibilities

• Coordinate person-centered plans for clients and families.
• Provide 175 units per month of face-to-face treatment per plan.
• Coordinate care with external organizations and courts as needed.
• Attend trainings and supervision; maintain licensure as required.
• Ensure confidentiality and adherence to MDHHS and CARF guidelines.
