Care Lync in Las Vegas, NV is seeking a full-time Case Manager to provide coordinated services for clients enrolled in Medicaid waiver programs. You will be highly organized and detail-oriented, building plans for each participant and updating them as needs change.

Qualifications

• BS/BA degree in related course work.
• No prior experience required.
• Experience with Microsoft Office, state-maintained databases.
• Maintain proficiency in company sponsored trainings and certifications.
• Maintain updated state related Clearances.
• Valid Driver's License including personal vehicle insurance coverage and current vehicle registration.

Responsibilities

• Coordinate care by assessing participant needs and creating individualized plans.
• Update plans annually or after significant medical or social changes.
• Communicate with participants, families and service providers to ensure seamless coordination.
• Develop and maintain professional relationships with participants and families.
