Job summary

Baystate Health is hiring a per diem Case Management Assistant at Baystate Medical Center in Springfield, MA. The role collaborates with patients and Case Managers to document accurate clinical and regulatory information and to coordinate care across teams.

You will support financial advocacy, communicate Medicare requirements, and assist with follow-up appointments, post-acute planning, and liaison with insurers, home care agencies, SNFs, and transportation providers.

Qualifications

• High School Diploma or GED required; Associate's degree preferred.
• Two to five years of health-related work experience.
• Associates degree in human services preferred.
• Insurance/payer experience preferred.
• Basic knowledge of medical terminology.

Responsibilities

• Collaborate with patients and Case Managers to document accurate clinical/regulatory information.

• Audit charts for Medicare requirements and communicate regulations.
• Assist with follow-up appointments and post-acute/transitional planning care needs.
• Support coordination with insurance providers, ACOs, home care agencies, SNFs, DME companies, and transportation agencies.

What you will do:

• The Case Management Assistant will work collaboratively with patients and Case Managers to communicate and document accurate clinical and regulatory information.
• Collaborate with internal and external partners to address financial advocacy for the patient/organization.
• Audit Charts for Medicare requirements and communicate regulations effectively
• Assist with follow-up appointments and telephone calls and other post acute/transitional planning care needs.
• Support coordination with representatives from insurance companies, accountable care organizations, home care agencies, skilled nursing facilities, durable medical equipment companies, and transportation agencies.

What you will need:

• High School Diploma or GED requires. Associate's Degree preferred.
• Two years to five years prior experience in a health related field.
• Associates degree in Human Services Field preferred.
• Insurance/payer experience preferred.
• Basic knowledge of Medical Terminology.

What you need to know:

• This is a per diem position. Must be available to work between 8:00 AM- 4:30 PM, Monday through Friday, and 6 weekends a year
• This position at Baystate Medical Center- 759 Chestnut Street, Springfield.

Note: The compensation range(s) in the table below represent the base salaries for all positions at a given grade across the health system. Typically, a new hire can expect a starting salary somewhere in the lower part of the range. Actual salaries may vary by position and will be determined based on the candidate’s relevant experience. No employee will be paid below the minimum of the range. Pay ranges are listed as hourly for non-exempt employees and based on assumed full time commitment for exempt employees.

$20.79 - $23.90 - $28.08
