Job summary

Universal Health Services' Compass program seeks a dedicated Case Manager to coordinate aftercare for clients based on an individualized treatment plan and a multidisciplinary team approach. You will mentor case management staff, connect clients to community and military resources, and support transitions toward discharge as stability improves, maximizing independence and well-being.

Qualifications

• Bachelor's degree required; Master's degree preferred.
• Minimum of 2 years of experience in case management.
• Completion of Crisis Prevention and Intervention (CPI) training within 30 days of employment

Responsibilities

• Coordinate aftercare needs of clients based on an individualized treatment plan.
• Mentor case management staff and collaborate with the multidisciplinary team.
• Assist clients in accessing community and military resources.
• Support transition toward discharge as stability improves.
