Position: Adult Mental Health Case Manager

RBH is seeking full-time Adult Mental Health Case Managers to perform the following duties:

  1. Assess client needs, develop, implement, and monitor service and assistance plans.
  2. Coordinate and monitor services with other agencies.
  3. Counsel and assist clients, maintain records and files, and prepare reports.
  4. Perform related work as assigned, under the supervision of the Program Supervisor.
Essential Functions
  1. Assess client service needs, capabilities, and appropriateness for services; present options based on a needs assessment.
  2. Develop, review, and update individual service plans with client/family consensus.
  3. Make referrals and linkages to appropriate agencies; coordinate services with multiple providers.
  4. Evaluate service quality, client condition changes, and environmental safety; analyze data and resolve problems.
  5. Plan and facilitate group sessions to promote recovery and educate clients.
  6. Assist clients with activities of daily living, including hygiene.
  7. Orient and train new staff; evaluate staff performance as directed.
  8. Serve as liaison to public agencies and provide information on programs and services.
  9. Participate in interdisciplinary team meetings; assist with discharge planning and transportation.
  10. Provide crisis intervention and prevention services.
  11. Prepare reports and maintain client records and charts.
  12. Attend meetings, staffings, conferences, court hearings, and testify as needed.
  13. Assist in curriculum development and resource tool creation.
