Job summary

Total Care Services, Inc. is seeking a Case Manager for Adult Medical Day Care Services (MDCSW) to support individuals with complex medical and functional needs working in community settings.

You will provide person-centered case management to individuals 16+ who meet nursing facility level of care but choose to live in the community, coordinating services and documenting care throughout LTSS Maryland.

Qualifications

• Bachelor’s degree in Human Services or related field (or approved equivalent).
• Some experience in case management or service coordination.
• Strong communication, documentation, and organizational skills.
• Valid driver’s license, reliable transportation, and safe driving record.
• Ability to travel locally throughout Maryland.
• Must be at least 21 years old.
• Ability to pass background checks.
• Authorized to work in the United States.

Responsibilities

• Conduct assessments using Person-Centered Planning (PCP).
• Develop and manage individualized service plans.
• Coordinate Adult Medical Day Care and community-based services.
• Document all service coordination activities in LTSS Maryland daily.
• Conduct in-person client visits.
• Monitor service effectiveness and client outcomes.
• Ensure compliance with program and regulatory requirements.
• Maintain accurate and timely documentation.

• Quarterly Bonus: Up to $1,250 based on performance
• Public Service Loan Forgiveness (PSLF) eligible employer
