Job summary

Safe Place for Youth in Venice, CA, is hiring a Case Manager for the Access Center to support case management operations, conduct intakes, refer youth to housing, health, education, and employment services, and maintain HMIS data. The role emphasizes crisis intervention, advocacy, and coordination with external agencies to promote youth wellbeing.

This is a full-time, on-site position with a 40-hour work week and a driver requirement.

Qualifications

• 1–2 years of experience in social services or related fields.
• Strong communication and interpersonal skills.
• Ability to work in a fast-paced environment and collaborate with multiple partners.

Responsibilities

• Support Access Center Case Management Team in day-to-day operations and programming.
• Run Front Desk operations on a rotation basis and connect members with needed services.
• Conduct intakes, assess needs, and refer youth to housing, mental health, benefits, education, and employment.
• Provide mediation and crisis intervention as needed.
• Develop youth-driven treatment plans, case summaries, and progress reports.
• Utilize HMIS and SPY Database to track data and prepare monthly reports.
• Attend external meetings with government agencies and community groups to advocate for members.
• Support housing and health and wellness goals for members.
• Collaborate across departments and support SPY programming.
• Follow legal and ethical guidelines.
• Perform other duties as assigned.
