Position Summary
Provide case management service to clients as outlined in the case management procedure manual in accordance program guidelines.
Essential Responsibilities
- Provide a minimum of 115 hours of service for assigned clients each month.
- Complete client interview and submit written findings and recommendations to referral source.
- Coordinate the provision of mental health treatment, competency restoration training, residential care or housing with supervision, medical and auxiliary services if appropriate, case management and monitoring of individuals who are being diverted from commitment to the Department of Children and Families.
- Attend court hearings in the cases of individuals being discharged from the state treatment facilities and ensure effective linkage to service continuum.
- Coordinate discharge planning activities for clients as assigned.
- Participate in treatment team and discharge planning meetings for clients as assigned.
- Develop and maintain a written assessment and service plan for each client with as much participation from the client as possible and reasonable.
- Develop and maintain Service plans in accordance with the agency protocol and procedures for frequency and timeliness for completion of this document.
- Ensure Service plans and assessments are appropriately signed and properly credentialed.
- Prepare written documentation of each service conducted and ensure documentation is performed in accordance with the agency’s standards for service documentation in the client\'s medical record.
- Ensure correctly completed service activity logs with progress notes attached are submitted to the finance department on a daily basis.
- Ensure progress notes accurately reflect the nature of service provided, activities provided, and the timeliness of services and outcomes.
- Attend court hearings in the cases of individuals and ensure effective linkage to service continuum.
- Review required monthly monitoring reports to intervene in problematic situations, provide alternative treatment modalities when necessary, and identify trends to improve service delivery.
- Participate in case conferences.
- Complete all orientation and mandated training activities, including E-Learning Courses, within required timeframes and in accordance with criteria for this position.

Qualifications
Certifications/Licensure
- Must be a Certified Behavioral Health Case Manager through Florida Certification Board or become certified within 1 year.
Education
- Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology or related social science field is required.
Experience and Competencies
- One (1) year of experience in providing services to target population in an in/outpatient setting is preferred.
- Intake and assessment experience is preferred.
- Bilingual is preferred.
- Ability to converse with consumers/families about substance and mental health issues.
- Ability to communicate appropriately and effectively with members of a multidisciplinary team.
- Ability to develop and maintain client rapport.
- Ability to maintain current knowledge of community social referral sources.
- Ability to maintain up-to-date knowledge of Medicaid/Medicare and Social Security disability application procedures.
- Ability to organize, interpret and communicate data effectively.
- Ability to network with internal and external departments and agencies.
- Knowledge of case management techniques and life span development.
- Knowledge of trauma, exposure to violence, substance abuse and mental health disorders and their inter-relationship.
- Ability to assess and develop appropriate service plans within a developmental framework.
- Ability to chart results of assessments, service plans, progress notes, discharge plans and other consumer information.
- Maintains confidential files and records; working knowledge of behavioral health terminology; knowledge of HIPAA, ethics, consumer rights and confidentiality; familiarity with WestCare services and programs.
- Understanding of cultural identity, ethnic background, age, development, gender and sexual orientation in prevention, intervention and treatment; demonstrates ability to work with culturally diverse staff and consumers and to respect cultural values.
- Articulates a working definition of trauma, understands its impact and re-traumatization.
- Demonstrates dependability, good attendance, flexibility, and ability to work independently with minimal supervision.
- Demonstrates ability to establish and maintain effective working relationships with consumers, co-workers, external partners, and regulatory agencies.
- Courteous, tactful, cooperative, and maintains a positive attitude; adheres to ethical standards and confidentiality; CPR/First Aid and annual TB test as required.
Working Conditions
- Work is primarily performed in an office or residential treatment setting; some outdoor activities may be required.
Essential Physical and Mental Demands
- Physical Demands: mobility and physical activity, extended sitting, lifting up to 50 pounds, use of computer and communication devices; ability to de-escalate physically when necessary.
- Talking, hearing and seeing requirements to perform duties and observe client behavior.
- Mental Demands: ability to analyze data, work under stress, and collaborate with diverse stakeholders; ability to use de-escalation techniques.
