Job summary

WestCare Foundation is seeking a full-time STOP Program Case Manager in North Highlands, CA. You will handle a large caseload, perform extensive documentation, and coordinate services across several agencies.

Travel to providers and in-prison presentations are required to support successful outcomes. Ideal candidates have two years in criminal justice settings and a four-year degree preferred; high school diploma is required.

Qualifications

• Minimum two years’ experience with criminal justice population.
• One year case management experience preferred.
• High School diploma or equivalent required.
• Graduation from accredited four-year college preferred in Addiction Counseling, Criminal Justice, Social Work, Psychology, or related field.

Responsibilities

• Case management of a large caseload in the STOP Program.

Position Summary

Person in this position will be responsible for the case management of a large caseload of participants in the STOP Program. Essential Job Functions include those listed below.

Essential Job Functions

• Case management of a large caseload of participants released from prison substance abuse programs.
• Heavy documentation.
• Coordination of various services throughout several agencies.
• Travel to Community Based Providers in personal vehicle.
• In-prison presentations.
• Parole collaboration.
• Must embrace and embody the mission, vision, guiding principles, clinical vision, and goals of WestCare Foundation.
• Perform all other duties as assigned.

• Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required.
• Must be able to obtain and maintain CDCR Clearance.
• CPR Certification, First Aid Certification, and an annual Tuberculosis test.
• During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.

Education and/or Experience

• Minimum two (2) years’ experience working with the criminal justice population.
• One (1) year case management experience is preferred.
• Graduation from an accredited four-year college with a degree in Addiction Counseling, Criminal Justice, Social Work, Psychology, or a related field is preferred.
• High School diploma or its equivalent is required.

Working Conditions

Work is performed primarily in an office, residential, or outpatient setting. Regional travel is required.
