Join our Viroqua Community Support Program team as a full-time Case Manager!
BASIC PURPOSE: To provide case manager services to persons with severe and persistent mental illness in the CSP program which allows them to live successfully in the community and grow their recovery.
ESSENTIAL DUTIES & RESPONSIBILITIES:
ESSENTIAL KNOWLEDGE, SKILLS & ENVIRONMENT:
Education, Training & Work Experience: Required is a bachelor’s degree in a behavioral science or a related field with 1,000 hours of supervised post-degree clinical experience with chronically mentally ill persons, or a bachelor’s degree in a field other than behavioral sciences with 2,000 hours of supervised post degree clinical experience with persons with chronic mental illness. Preferred is a master’s degree in social work, clinical psychology or psychiatric mental health nursing or having met equivalent educational requirements with 3,000 hours of supervised clinical experience working with persons with severe and persistent mental illness outside of a CSP program or 1,500 hours of supervised clinical experience working within a CSP program is preferred. A bachelor degree in a behavioral science or a related field with 1,000 hours of supervised post degree clinical experience with persons with severe and persistent mental illness or a bachelor degree in a field other than behavioral science with 2,000 hours of supervised post degree clinical experience with persons with severe and persistent mental illness.
Specialized Knowledge & Skills: Familiarity with the human service delivery system, DHS 63, recovery principles, and appropriate ethics and boundaries.
Equipment & Applications: Ability to use Microsoft Office programs, e-mail, scanner, and FAX machine, as well as enterprise software. Driver’s license and ability to use own vehicle to provide client transportation.
Work Environment & Physical Demands: Indoor office environment, as well as at least 50% of work in a community setting or in client’s homes. Local travel will be required. Additional physical demands could include occasional lifting, bending, twisting, or reaching.
