Job summary

Granite Hills Hospital is seeking a full-time Case Manager to coordinate discharge planning and develop patient-centered plans for post-hospital needs. The role involves ensuring continuity of care and a day shift schedule with 40 hours per week and full benefits.

The successful candidate will have a Master’s degree in Social Work or related mental health field, and a minimum of three years’ psychiatric health care facility experience in case management and discharge planning, plus crisis

Qualifications

• Master’s degree in Social Work or a clinical related mental health field preferred.
• Minimum of three (3) years’ experience in a psychiatric health care facility with direct experience in Case Management and discharge planning.
• Crisis Prevention Certification trainings (Handle with Care) within 30 days of employment.

Responsibilities

The Case Manager is responsible for planning and coordinating activities related to discharge planning. Developing and implementing a plan of care designed to meet the patient’s post hospitalization needs while ensuring continuity of care.

Qualifications

Education:

Master’s Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.

Experience:

Minimum of three (3) years’ experience in a psychiatric health care facility with direct experience in Case Management and discharge planning.

Additional Requirements:

Crisis Prevention Certification Trainings (Handle with Care) within 30 days of employment and prior to assisting with a restraining procedure. Certification training is offered during new hire orientation.
