Job Summary:

The Respite Case Manager provides direct services to adult Department of Mental Health clients in short-term transitional clinical and rehabilitative services, both in residential settings and in the community. Responsibilities include assisting persons in setting and achieving individual goals, supporting stability and transition, and maintaining daily residential operations. The role involves medication administration and documentation, requiring organizational efficiency and a person-centered, strengths-based approach.

Supervisory Responsibilities:

None

Essential Functions:
  1. Build relationships with Persons Served to manage symptoms and stressors, supporting their ability to remain or transition to stable living environments.
  2. Provide direct support and case management in residential and community settings.
  3. Assist in writing and achieving treatment and rehabilitative goals, including housing and community supports.
  4. Organize activities and outings aligned with treatment goals.
  5. Maintain cleanliness, assist with meals, and ensure safety protocols are followed.
  6. Complete documentation such as IAPs, service notes, incident reports, and outcome measures.
  7. Assist in assessments and crisis interventions.
  8. Administer medications following MAP standards and train clients in medication self-administration.
  9. Maintain relationships with community resources and partners.
  10. Perform other duties as assigned.
Qualifications:
  • Minimum 2 years’ experience in a mental health setting; BA/BS preferred.
  • Valid driver’s license and ability to travel throughout Metro Boston.
  • Department of Public Health certification in medication administration (MAP) required within 60 days.
  • Strong communication and organizational skills.
  • Proficiency in computer and internet use.
