Qualifications

• Must have a high school diploma and one to two years of related experience and/or training.
• Familiarity with documentation and reporting in social service programs.
• Computer-literate; able to operate SERV Tracker and standard software.

Responsibilities

• Educates the community on homebound meal outreach programs and their benefits.
• Distributes educational materials in the community.
• Visits homes to assess client needs and develops plans to meet them.
• Monitors clients for the homebound meal program and documents progress.
• Completes required paperwork in compliance with state and program regulations.
• Keeps accurate records in SERV Tracker and related files.
• Administers initial nutritional screenings via questionnaires.
• Helps clients obtain necessary resources and referrals as needed.
• Refer clients to the Director of Nutrition Services for counseling when appropriate.
• Documents client cases and assessments; prepares program reports.

CASE MANAGER (PT)

Monday – Friday 9:00 am to 3:00 pm

(Paid vacation and sick time with a supplemental insurance package)

SUMMARY

Screens and assesses the eligibility of elderly candidates for participation in the Homebound Meal Program.

ESSENTIAL DUTIES AND RESPONSIBILITIES

• Educates the community on various homebound meal outreach programs and the importance of utilizing the program
• Distributes educational materials in the community
• Visits homes to determine client needs, develops a plan to meet needs
• Identifies and monitors clients for homebound meal program
• Completes required paperwork in compliance with state and program regulations
• Creates and maintains records in SERV Tracker software
• Administers initial nutritional screening in the form of a questionnaire
• Helps client obtain necessary resources
• Refers clients with social and emotional problems to other community agencies for assistance
• Refer clients to the Director of Nutrition Services for nutrition counseling as needed
• Documents client cases, including assessments and reassessments, and maintains records on each client
• Prepares reports as required by various federal, state, and local programs
• Helps organize and participate in fundraisers benefiting the program

CERTIFICATES AND LICENSES

• Valid driver’s license required
