Job summary

Spread Your Wings LLC in California seeks a Case Manager to lead a team delivering supportive living services to individuals with intellectual or developmental disabilities in Lake County. This is not a remote position and requires reliable transportation and county-area travel.

Schedule: Wednesday to Sunday with on-call coverage for emergencies. You will develop partnerships with Regional Center staff, strive for top-quality service delivery, maintain confidentiality, and ensure audit readiness

Qualifications

• Minimum of 1 year of management experience in assisted living overseeing staff across multiple units.
• Proficient in Microsoft Office; ability to generate and analyze reports.
• Experience collaborating with Regional Center staff is a plus.
• Ability to maintain confidentiality and handle sensitive information.

• Develop and nurture partnerships to enhance service delivery.
• Support a culture of high-quality service and client rights.
• Build strong internal relationships and effective employee relations.
• Assist in keeping the branch audit-ready at all times.
• Perform other duties as assigned.

The Case Manager supports the Supportive Living Services for intellectually disabled clients living in Lake County.

Schedule Wednesday - Sunday - (Must be available to cover Monday and/or Tuesday for emergencies

• Assisting to keep your branch audit ready at all times
• Performs other duties as assigned

Qualifications
