Qualifications

• Highschool diploma or GED required; associates degree or higher preferred in behavioral health.
• 6 months recovery from substance use and/or mental health preferred.
• 1-3 years of GMH/SU experience required.
• 1-3 years in behavioral health outreach/engagement.
• 1-3 years in medical setting preferred.

Responsibilities

• Provide case management in community and/or office settings.
• Assist patients and care team in planning, connecting, outreach, advocating, coordinating, and monitoring needs.
• Assist patients in gaining access to health services, financial assistance, employment, education, social services and supports.
• Assess patient needs, develop and monitor individualized treatment plans, document services timely.
• Promote recovery philosophy and a safe recovery environment.

Job Summary

The Case Manager provides case management services in either the community and/or in office setting to patients and their families. The Case Manager assists the patient and care team in planning, connecting, outreaching, advocating, coordinating and monitoring the patient’s needs and progress. The Case Manager assists patients in gaining access to health services, financial assistance, employment, education, social services and natural supports. The Case Manager assesses patient needs, develops and monitors individualized treatment plans and documents services provided in an accurate and timely manner. Embraces the recovery philosophy and promotes a safe recovery environment for clients.

Skills/Requirements

• Highschool diploma or GED required. Associates Degree or higher in a field related to behavioral health preferred.
• Minimum of six month of recovery from substance use and/or mental health disorders preferred.
• 1-3 years of experience working with the GMH/SU population required.
• 1-3 years of experience in behavioral health position providing outreach and engagement activities required.
• 1-3 years of experience in a medical setting preferred
• Valid identification required
• Current AZ Driver's License (valid and in good standing), preferred.
• 39-month MVR preferred
• Behavioral Health Technician (BHT) in accordance with A.A.C. R9-10-101.33 and CBI Clinical Policies and Procedures
• Arizona Fingerprint Clearance card
