Riverside Recovery Of Tampa Llc in Tampa, FL is seeking a dedicated Case Manager to ensure discharge plans are appropriate, coordinated and followed from admission through aftercare. You will liaise with the clinical and medical teams, develop relationships with external providers, document in patient records, and link clients to community resources.

Qualifications

• BA/BS degree in Human Services, Healthcare, or related field.
• One year of discharge planning or case management experience.
• Knowledge of EMR software such as Kipu.
• Strong oral and written communication skills.

Position Summary:The Case Manager is responsible for ensuring the clients discharge plans are appropriate, coordinated and are followed through from admission through all levels of care and thereafter. This position will act as a continuum specific liaison and resource for alumni thereby fostering long-term relationships with the organization. Riverside Recovery of Tampa provides a multi-directional discharge plan designed to continue the patient's successful journey of recovery and well-being.Essential Duties and Responsibilities:To perform this job successfully, an individual must be able to perform the following satisfactorily; other duties may be assigned. Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions.Coordinate with clinical team, medical team and patient to identify needs following services at Riverside RecoveryCoordinate admission/intake/appoints with follow-up servicesDevelopment ongoing relations with external providers in various fieldsFacilitate groups as assigned and deemed competentDocumentation in patient recordsCorrespondence with external entitiesRefer clients to local community health entities (psychiatrist, therapist, Primary Care Physician, etc.)Obtain medical records; Marchman Court documents; and FMLA/STD informationAssist in facilitation of the discharge process for each client and assist with aftercare planningUpdate discharge documentation daily to meet all Joint Commission and DCF requirementsAttend daily billing/admission/clinical team meetingsUpdate tracking spreadsheet on daily basisAssist clients in obtaining necessary resources to achieve case management goalsMaintain client related data systems, including case notes and complete discharge planning entriesAdheres to strict boundaries and professional ethics in the care of othersAssist in facilitation of the discharge process for each client and assist with aftercare planningLink clients to community resources, and develop relationships throughout the communityEthical and professional standards in compliance with company policiesMust be able to develop a network of community resources as it applies the substance abuse and addiction treatment industryOther Duties as AssignedFull time Eligible Benefits:· Health Insurance· Dental· Vision· 401- K· Life Insurance· Disability· Critical Illness· Hospital Indemnity/Accident plan· Legal planEmployer Paid Benefits:· EAP· $10,000 life insurance· $10,000 Short term DisabilityQualifications:BA/BS Degree in Human Services, Healthcare, or related fieldOne year of experience as discharge planner or related position (i.e., Case Manager)Knowledge and use of EMR Software; Kipu, etc.A general knowledge and background in substance abuse/behavioral healthAbility to develop rapport with patient, family, support system and external resourcesMust have good oral and written communication skills
