Job summary

Brooks Rehabilitation is seeking a skilled Case Manager to coordinate care for inpatients and drive effective discharge planning. You will lead a team of case managers, develop creative discharge plans, and ensure utilization criteria are applied to each case.

Strong communication and leadership will help connect patients with local resources for community reentry. The role requires clinical credentials (RN or social work certifications) and experience in acute or inpatient rehab, with a focus

Qualifications

• RN, MSW, LCSW, CRC, RN, CCM with Bachelor's degree minimum.
• 1 year of Case Manager or Social Worker experience in Acute Care or Inpatient Rehabilitation.
• Experience with brain injuries or medically complex patients with chronic needs.
• Strong communication to coordinate rehab teams and families through discharge planning.

Responsibilities

Job description

The Case Manager is the facilitator of the case and the team working with the patient. Strong leadership and facilitation skills. Ability to work with all stakeholders involved in cases to support a positive experience and outcome on each case. Knowledge of local resources. Ability to develop creative discharge plans to support patients in the inpatient rehabilitation setting. Understanding of criteria and ability to apply for utilization management of cases.Qualifications:RN, MSW, LCSW, CRC, RN, CCM with Bachelor's degree as a minimum.1 year experience as a Case Manager or Social Worker in Acute Care or Inpatient Rehabilitation.Experience working with patients and families that have acquired brain injuries or with Medically Complex Patient Populations that have chronic disabilities and multiple social and medical community resource needs.Strong communication skills to facilitate rehabilitation teams and families through complex discharge planning with the goal of community reentry – i.e. manage chaos.Individual must be accountable for the scope of their work and able to work independently to manage their caseload as well as with a team of 12 Case Managers to ensure the entire hospital receives needed Case Management services.Ability to delegate tasks to ensure efficiency in their day to day casework.Knowledge of or ability to research community resources both locally and in geographic areas beyond Jacksonville.Must be a team player and have strong leadership skills to facilitate discharges of complex patients back to their communities.Desired Qualifications:Complex Case Management, Discharge Planning, and Placement experience preferred.Experience with insurance and knowledge of Medicare and Medicaid benefits preferred.Demonstrate an ability to network and develop key relationships needed to support successful discharges.Shift: 40 Hours per weekLocation: 18500 64th Street, Phoenix AZ, 85054Compensation: Experience, education and tenure may be considered along with internal equity when job offers are extended.Thriving in a culture that you can be proud of, you will also receive many employee benefits such as the following:Competitive PayComprehensive Benefits packageVacation/Paid Time OffRetirement PlanEmployee DiscountsClinical Education and Professional Development Programs
