Stony Brook Medicine is seeking a Case Manager to enhance patient management, maximize satisfaction, and promote cost effectiveness. You will oversee the full patient care cycle with professionalism and organization.

Qualifications

  • Comprehensive knowledge of mental health disorders and the social services system.
  • Case Manager Certification
  • Excellent knowledge of case management principles, healthcare management and reimbursement.

Responsibilities

  • Coordinate and provide care that is safe, timely, effective, and equitable and client centered.
  • Screening of patient seeking care in or being referred to the Outpatient Department.
  • Handle case assignments, draft service plans, review case progress and determine case closure.
  • Help patient to achieve wellness and autonomy.
  • Facilitate case coordination and information sharing.
  • Assist patients in making informed decisions by acting as their advocate regarding clinical status and treatment options.
  • Develop effective working relations with providers to build out the entire treatment plan, designed to get patients back into a productive, healthy place in their lives.
  • Prior authorizations for medications (submit appeals if necessary)
  • Submit Health Home referrals for long term case management
  • Assist and complete referrals to partial hospitalization programs/inpatient/LINK program/IOP programs/PROS programs/Vocational services
  • Assist with applications for patients and conduct psychosocial if necessary
  • Conduct psychosocial needs assessments and individual and/or group-based psychotherapy services
  • Participate in social work residency trainings
  • Other duties assigned by management

Days/Hours: Monday - Friday; 8:30 AM - 5 PM

Pay: $28.95 - $36.17

Individual compensation discussions begin during the hiring process and may occur during job review and promotional opportunities. Salaries vary depending on experience, education and current market for the position. Human Resources determines the external and internal equitable salary for each employee.

Responsibilities

SUMMARY: The Case Manager is responsible for enhancing the quality of patient management, maximizing satisfaction and promoting cost effectiveness. The successful candidate will be responsible for the full patient care cycle. The incumbent is expected to demonstrate a high level of professionalism, organization, implementation/evaluation of actions required to meet the patient's health and human services needed.

Physical Demands:

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. While performing the duties of this job, the employee is regularly required to communicate with patients, staff and medical providers. The employee must be able to exchange accurate information in these situations. This position is active and requires the employee to constantly move about in the office to assist patients, staff and medical providers. Any additional physical demands will be outlined and provided by management.
