Position Details
The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. The Case Manager provides intensive therapeutic services in various settings, including community living arrangements, schools, community settings, natural support residents, etc. This role involves collaboration with the treatment team to implement and transfer skills based on a person-centered approach.
