Job description

JOB DESCRIPTION: CASE MANAGER

REPORTS TO: Program Director

CLASSIFICATION: Full-time; non-exempt.

MISSION

Jodi House empowers brain injury survivors to not merely survive but thrive by providing support services that encourage their physical, cognitive, and behavioral recovery, regardless of the ability to pay.

POSITION SUMMARY

Under the direction of the Program Director, the Case Manager provides comprehensive, individualized case management and support services to adult brain injury survivors in Santa Barbara County and beyond. The Case Manager works closely with our members to assess their unique needs, develop personalized recovery plans, and coordinate a range of services to help maximize independence and enhance their overall quality of life. Through the provision of these personalized services, this position is high-impact and integral to Jodi House’s mission.

ESSENTIAL JOB FUNCTIONS

  1. Provide one-on-one case management support to Jodi House members in furtherance of their individual recovery goals, including developing, evaluating, and revising case management goals and plans.
  2. Navigate a wide variety of resources that will help both in-person and remote members achieve their goals and improve quality of life.
  3. Assist members in aspects of independent living that include the development of social, vocational, and effective life management skills.
  4. Recruit, interview, train, and supervise research and case management interns.
  5. Develop trust and rapport with survivors in a linguistically and culturally appropriate ways, utilizing available resources including translation services when needed.
  6. Recognize and assist members in emotional distress in developing appropriate coping and conflict resolution skills.
  7. Function as and fulfill all requirements of a mandated reporter.
  8. Educate members and their families/caregivers about brain injury and effective symptom management.
  9. Assist members in building healthy peer and familial support networks.
  10. Facilitate classes, support groups, and the Immersion Services pre-vocational program as needed.
  11. Monitor home safety of members, conduct home visits as needed, and make appropriate referrals to community resources.
  12. Monitor member safety at all times and take appropriate and timely action in response to urgent or emergency situations.
  13. Educate members in finding alternative community resources when the need arises and assist with the completion of necessary paperwork.
  14. Serve as a representative of Jodi House to the community and collaborate in various community partnerships.
  15. Work with highly sensitive information and maintain confidentiality.
  16. Collect and document program data and services in member database, CIL Management Suite, and prepare reports in a timely manner.
  17. Oversee Jodi House’s survey and data collection and reporting process, ensuring compliance with requirements established by the Department of Rehabilitation and other granting agencies.
  18. Assist with special events as needed.
  19. Other duties as assigned.

What You’ll Need

  1. MSW, ASW, LCSW, MFT, or LMFT.
  2. Case management experience, preferably in working with adults with disabilities.
  3. Knowledge of local (Santa Barbara) medical, health care agencies, and social service agencies, and demonstrate willingness to learn and navigate other county systems.
  4. Experience collaborating with volunteers or interns.
  5. Bilingual Spanish/English language abilities preferred.
  6. Experience or education related to working with adults with disabilities.
  7. Excellent interpersonal and customer service skills with ability to communicate effectively, both in writing and verbally.
  8. Ability to organize and prioritize work and work independently.
  9. Ability to remain calm and take appropriate actions in stressful or emergency situations.
  10. Knowledge of relevant computer programs and other office equipment.
  11. Current CPR/First Aid certification, or willingness to obtain within 60 days of hire.
  12. Current California Driver’s License and proof of insurance.

SCHEDULE

Position is full-time, 40 hours per week, and non-exempt; some nights and weekends may be required. Business hours are 9am-5pm, Monday-Friday. This position will report on-site to Jodi House at 625 Chapala Street, Santa Barbara, CA 93101. This is not a remote work opportunity. The full-time position will start in May 2025, with shadowing and training opportunities starting in April 2025.
