Job summary

LanceSoft Inc. is seeking a Human Services Case Manager to provide comprehensive healthcare management services in Southwest Dallas. The ideal candidate will have a bachelor’s degree in a relevant field, bilingual skills preferred, and at least two years of case management experience.

Estimated Length of Assignment: 06+ Months With Possible Extension

Description:

This role is working remotely. This position will require travel to meet in person with members around Service Delivery Area 75% of the time.

Travel Support: Mileage reimbursement is provided.

Location: Southwest Dallas - Grand Prairie, Arlington, Mansfield

· Provide comprehensive healthcare management services to facilitate appropriate healthcare treatment, effectively manage healthcare costs, and improve healthcare program/operational efficiency involving clinical issues.

· Determines medical necessity/appropriateness.

· Facilitates optimal outcomes.

· Identifies and follows through with continuous quality/compliance opportunities. May also include identification of aberrances and initiation of corrective action.

· Educates/empowers customers to ensure compliance, satisfaction, and promote patient advocacy.

· Optimizes total costs.

· Implementation and evaluation of policy based on usage and program directives.

· Educates/empowers colleagues at all levels to enable decision-making at the most appropriate level

Position Summary:

Through the use of service coordination tools and information/data review, conducts comprehensive assessments of referred member’s needs/eligibility and determines approach to case resolution and/or meeting needs by evaluating member’s benefit plan, and available internal and external programs/services .Completes face to face visits and monthly phone contact with members, utilizing various treatment modalities and interventions to achieve desired goals for members. Applies judgment to the incorporation of strategies designed to reduce risk factors and address indicators which impact care planning and resolution of member issues while aware of member diagnosis. Utilizes motivational interviewing skills to ensure maximum member engagement and promotes lifestyle/behavior changes to achieve optimum level of health

Schedule:

Monday- Friday 8a to 5p
