Job Summary:Under the direction of the Program Supervisor, we are seeking a full-time, qualified Case Manager to provide rapid rehousing to eligible individuals who are justice-involved, as well as individuals and families, through the county’s homeless Continuum of Care (CoC) and Coordinated Entry System (CES), in compliance with CoC Program Models and Performance Standards. Rapid Rehousing Case Manager helps homeless individuals and families find and maintain stable housing. The Case Manager will connect clients with resources, develop housing plans, and provide ongoing support to ensure long-term housing stability. This role often involves close collaboration with other social service agencies and landlords. The ability to maintain a thorough understanding of the CalAIM program and leverage cross-sector partnerships to support positive housing and health outcomes for participants.
